Health and Safety Inspections
What is a Health and Safety Inspection?
Most child care providers offer safe and healthy environments. Health and safety inspections and investigations are a normal part of child care in Idaho. Inspections are not intended to "catch" anyone doing something wrong. Inspections help ensure the safety of children and staff, support staff in following requirements by finding concerns, and promote continuous improvement.
Idaho began publishing the results of health and safety inspections October 2017. Inspections are published as they are conducted and maintained on this site for at least three years.

Incident Reports
What is a Substantiated Incident?
Occasionally, families and community members may report that a child care facility is not meeting requirements for safe and healthy child care. All child care incidents reported in Idaho are investigated. After an investigation, if it is determined by the relevant authorities that an incident occurred, it is considered “substantiated.” It is important to know that only substantiated incidents and allegations are published on this website. Incidents that are still under investigation will not be published until the investigation is concluded.
Idaho began publishing substantiated incidents in October 2017. Incidents are published as each investigation is concluded and maintained on this site for at least three years.
Supervision concerns
There were concerns a child was left outside without proper supervision. The Health District completed an investigation and confirmed a child had exited the facility through the side door without the staff’s knowledge for approximately 30 minutes. Provider was advised of options to ensure the situation does not happen again such as periodic checks of all students inside and outside, adding a security alarm to the door, having scheduled outside time, and/or replacing the door with one that has the push pad device higher up where the children can’t access it.
Resolution
Provider added a gate to the exit door along with signage that states to keep the door latched and secured at all times.
Child Abuse/Neglect
There were concerns of an injury to a child due to a child being left in an an unattended vehicle. An investigation was completed and determined a child was left in a van alone for over 3 hours. Issue was forwarded for criminal action.
Resolution
Provider was found guilty of a misdemeanor for Childcare Facility Transportation Safety Violation - Unattended Children in a Vehicle. As of 8-13-23, Urban Green Kids - Fairview is out-of-business.
22-00055 Supervision Concerns
A child left the facility through the front door and none of the staff were aware of it.
Resolution
A bell has been installed on the exit door to ensure children are not able to leave the facility without staff awareness.
Lack of Supervision - Child-Staff Ratios
This complaint involves supervision concerns as child to child sexual abuse was reported and possible strangling of a child by a child.
Resolution
Investigation found couches that faced a wall that obscured the view of children. Couches were moved up against the wall so the entire room could be seen by employees regardless of where they were standing. Supervision practices were discussed in that children must be seen/heard at all times.
